"Tamang Proseso, Malinis na Dokumento: A Public Guide in the Registration of Birth" was formulated in order to introduce a more realistic approach to solve the increasing problem of the public regarding erroneous civil registry documents. The introduction of the solution finds its way after a careful assessment of the organization and analysis of the existing problem emanating from the enforcement of the civil registry functions of the agency and of the Municipal Civil Registry Offices nationwide. Particularly, this Re-entry Project sought to contribute in the improvement of the life of every Filipino citizen by providing the public with Certificate of Live Birth (COLB) that is free of errors or unwarranted entries through the introduction of the crafted Birth Information Sheet (BIS). The BIS was designed for self-administration because it contains the instructions on how the items are to be filled up. The BIS is purposely to extract accurate information from the informant-registrant and will be used as reference in the preparation of the COLB of the person registered. To further reinforce the main objective of the Re-entry Project, series of Information Dissemination on Civil Registration with Orientation on Proper Filling-up of the Birth Information Sheet (BIS) was conducted to expectant parents who are member-recipients of the 4P's, Barangay Health Workers (BHW's), Barangay Nutrition Scholars (BNS's), Chil Development Workers (CDW's), midwives and nurses of the Municipal Health Office including medical record officers of government hospitals. The Birth Information Sheet is an output of the workshop conducted in collaboration with officers and members of the Philippine Association of Civil Registrars-Nueva Viscaya Chapter. On the other hand, the information dissemination with orientation was successfully conducted in collaboration with the Local Government Unit of Bambang, Nueva Viscaya through the office of the Municipal Mayor, the Municipal Health Office, Municipal Civil Registrar's Office and the Municipal Social Welfare and Development Office. The result of the monitoring and evaluation conducted in the piloted barangays reveals that the current system of the providing health services in the barangay is the best avenue to introduce the use of the BIS. Civil registration services are made available and accessible when this service is integrated in the system of the frontline workers in the barangay level. Moreover, the use of BIS by pregnant mothers who subsequently delivered their child served the purpose of curbing the commission of errors during the preparation of the COLB as shown by the information and actual presentation of COLB gathered during this phase. The results and inputs gathered during the monitoring and evaluation paved the way for the development of a process flow for the effective use of the BIS. The successful implementation of the Re-entry Project is credited primarily to the concerted and coordinated effort of the Re-entry Project team members and the unwavering support, commitment and collaborative efforts extended by the officers and members of PACR-Nueva Viscaya Chapter and the Local Government Unit of Bambang, Nueva Viscaya.
