Barngay Dilam is a GIDA barangay in the municipality of Calayan. It is one of the barangay with the lowest facility based delivery in 2013 with 4.1%. The goal of this action plan and project is to increase facility based delivery in the barangay. This study specifically aimed to describe the current state of maternal health among mothers in the barangay. Another objective was to investigate gaps, needs, issues contributing to the persistently low facility based delivery. Through focus group discussion with the barangay officials, addressing issues and concerns, and lastly to recommend options to implement IntanMasikog Maternal Health Program in Barangay Dilam. Survey was conducted among forty respondents who were pregnant mothers and who delivered one year ago. Open ended questions were asked reasons why they deliver at home. Data were gathered and analysed, 60% under constructed barangay health station, distance is 17%, 10% for unnoticed active labor, 5% for delayed transportation, 4% for no savings and advised by husbands. Intensive IEC lectures and role plays were conducted to advocate maternal health care in the barangay and to increase awareness to all mothers the importance of giving birth in the barangay health facility. A Buntis baby bank Project was implemented in the barangay were pregnant mothers in their first visit will bring a bank made of bamboo where they will deposit their savings with a minimum amount of P 150.00/ month. It is for pregnant mothers to save money to spend during their delivery instead of paying traditional birth attendants with an amount of P 1,000, they will just spend their savings for the baby and their basic needs. Large spot map was created to monitor closely those far flung sitios with pregnant mothers. Every household in the spot map are pinned with colored pins. A barangay ordinance was implemented about pregnant mothers are prohibited to delivery at home. This was approved by the barangay officials. Transportation scheme was also arranged in the barangay. During completion of the birth plan, the midwife and the pregnant mother will designate an owner of a vehicle for her transportation to the health facility before or during labor. Preparedness throughout the pregnancy will be planned by completion of the birth plan during prenatal visits of pregnant mothers. These actions ultimately resulted to the increase in facility based delivery in Barangay Dilam form 4.1% in 2013, 24% in 2014 to 60.87% in 2015 from Januray to August. Recommendations on this study would be a waiting home must be constructed for pregnant mothers who resides in a far flung sitios. It will be constructed at the back of the health center. Standard operating procedures should be made to fully operationalize the facility particularly in pregnant mothers who will utilize the facility.
