In the local setting, the barangay health systems are weak evidenced by the absence of functional governance bodies and inequitable performance. The reforms for health governance should give emphasis to state actors, specifically the barangay public servants, because they know the specific health needs of their constituents. Hence, this study aims to improve barangay health systems by engaging the state actors towards good health governance. Survey, Document Review, Key Informant Interview and Focus Group Discussion were employed to assess the baseline status of the barangay health governance and the gaps; and, assess how engaging actors addressed those gaps. Results show that the poor health governance was caused by gaps in ensuring the Accountability, Participation, Predictability and Transparency of the barangay health systems. This was compounded by the dysfunctional linkages from the state actors to healthcare providers, from the healthcareproviders to state actors, and from the state actors to the citizens. In response to the identified gaps, the "SidlaKalusugan: Seal of Health Governance" was formulated which focuses on the engagement of health system actors through the Bridging Leadership framework, organization of Barangay Health Boards; and, a performance based incentive system. Analyses show that the project has addressed all the identified gaps and that the barangays with barangay health boards are performing better. Hence, this study has been successful in improving health governance in the barangays trough the state actor-focused interventions leading to better health outcomes.
