Thus, the assessment of community needs is vital in the development of a more responsive health system. Working with identified stakeholders and key players is essential in making this endeavor possible. Hence, this case study utilizes the Bridging Leadership approach which resulted to the development of an anti-hypertension program that will bring about change in the local health system. Data gathering techniques used to were Ocular Observation, Review of Records and Client Survey. Health Situation Analysis, SWOT analysis, Stakeholder Analysis and Multinomial Logistic regression analysis were employed in this project to identify local health system gaps and other factors that affects the increasing prevalence of hypertension in Divilacan, Isabela. The result of this study, developed a co-ownership of the problem of hypertension that further created a social response as evidenced by community participation through launching of Barangay Health Club and strengthening the screening anad monitoring of patients which is now in its implementation phase. Hence, this case study has been successful in establishing a collaborative partnership with the commitment and support of various stakeholders and key players in the community. ER -
