The country is indeed facing unique challenges in aligning its health system with the needs of its constituents because of the many gaps. In the Municipality of Sultan Naga Dimaporo, there was a rising trend of Facility-based Delivery (FBD) but during year 2015, only 40.8% FBD was met which was still low compared to the national target. Among the 37 barangays of the municipality, two barangays, Barangay Pinakalawag and Barangay Ramain were noted to have contributed to a great number of home deliveries and maternal mortality. Hence, this paper aimed to increase FBD and improve maternal health outcome in Barangay Pinakalawag and Barangay Ramain, Sultan Naga Dimaporo, LDN. This study was a descriptive study with qualitative and quantitative data as output. Triangulation method of gathering data was used which includes Data analysis, Focus Group Discussion (FGD), and Survey. The gaps were being identified using the health systems approach. Analyses showed that the continuing practice of TBAs was the cause of low FBD. Strategies were formulated to address the gaps of low FBD through the Action Plan and Project (APP) entitled "Yna, akong pinangga (YAKaP): A comprehensive health systems approach to increase facility-based delivery in Barangay Pinakalawag and Barangay Ramain, Sultan Naga Dimaporo, Lanao del Norte". Through this project, facility based delivery in the municipality as of 2nd quarter 2017 had increase significantly to 87.3% with an overwhelming increase also on FBD to the two barangays. Barangay Pikalawag with 81.8% and Barangay Ramain with 90.3% with no maternal mortality. Thus, this study has been successful in improving FBD in the municipality ER -
