The study aims to reduce maternal mortality in high risk pregnancies in the municipality of Claveria, Misamis Oriental. The research utilized the three-delay model in investigating for the factors contributing to maternal death. Qualitative analysis was done and the results can be summarized to Family/Community-based factors: (1) patient's preference, (2) financial issues, (3) lack of knowledge; and Health Service factors: (5) access to health facility (6) health personnel. Based on the identified problems, interventions were formulated and categorized according to the building blocks of health system. The FAST tool was used to identify the quick wins and key strategies. The strategy that received the highest score is the IEC on Safe motherhood, Birth and Family Planning and followed by the automated pregnancy tracking tool. The automated pregnancy tracking tool allow active tracking of high risk pregnancies. The tool reminds the health workers to actively track the pregnant patients and ensure delivery in the appropriate facility. The implementation of the tool showed improvement of the maternal health outcome and ultimately reducing maternal death in High risk pregnancies.
