Objectives: This study aimed to determine the causes or reasons of the shortage of doctors at the LGU health facilities that include Rural Health Units and Local Government Hospitals in Surigao del Norte, as basis for formulating a sustainable solution to address the prevailing issue. Methods: Municipal Mayors from 20 municipalities and 1 Mayor from the lone City of Surigao were respondents of the sruvey using the questionnaire. Key informant interview (KII) were conducted among some Municipal Health Officers (MHOs) and Chief of Hospital of the Province. The data that were gathered through the survey and KII were analyzed through quantitative and qualitative analysis. In parallel to the data of Surigao Del Norte, the data that were gathered through KII among MHOs and Provincial Health Officer II of the sister Province of Surigao Del Sur was used. The modified framework of analysis was utilized based on the Six Human Resource for Health Action Framework (HAF) of the WHO, USAID, Global Health Workforce Alliance was also utilized, such as; Leadership and Governance; Partnership; Finance; Education; Policy and Human Resource Management. Results: There are 21% of the MLGUs having issues on the retention and recruitment processes of all the municipalities in Surigao Del Norte. Out of the 20 MLGUs, 70% are interested to retain their DTTB but the physician decline. Specific reasons are; there is no applicant for MHO position, DTTB are not interested maybe due to Low salary and health budget. About 95.23% of MLGUs in the province are IRA dependent; they could not afford to provide adequate salary and other benefits to the MHO and other health personnel at pat with the neighboring Province. Conclusions: There is a need to pass a Provincial Ordinance Establishing a Scholarship Program for Medicine Proper as a long-term solution of the PLGU to address the issue on shortage of doctors at all LGU health facilities. So that, whoever is the elected governor of the province the program will continue to go on with the policy already in place. Substantial budget for medical scholarship of the province shall be embedded in the ordinance to ensure its sustainability either taken from the office of the governor or where it could be inserted in the different provincial offices. MLGU should appropriate certain percentage from their annual budget for their own student availing the provincial scholarship program to ensure return-service after passing the licensure examination.
