520 | _aLifestyle related diseases have indeed been increasing nowadays, affecting people not only in highly civilized and urban areas, but also in rural areas like in Llanera, Nueva Ecija. As of the latest data from the cases of hypertension and diabetes, as well as their complications. There have been several efforts made by the national government to address the problem on non-communicable diseases. However, these efforts still need thorough implementation not just in the local government units such as cities and municipalities, but more importantly, down to the farthest barangays. This Action Plan and Project is about organizing an inter-barangay-based hypertension and diabetes club, branded as ALDUB Club or the ALtapresyon Diabetes United Barangay Club in Llanera, Nueva Ecija,as a sub-group of the municipal-wide lub. This APP aims to help address the problem on compliance to maintenance medications in a low-resource rural area in Central Luzon. The researcher used Bridging Leadership Framework, wherein the community and its stakeholders were mobilized and became part of the changing process through the empowerment of several barangay ALDUB coordinators who, in the process, co-owned the community problem and later on became bridging leaders in their respective barangays, eventually becoming catalysts for community change. In conclusion, the organization of an inter-barangay-based ALDUB Club has improved the general health condition of the Llaneros through regular monitoring of blood pressure and timely supply of free maintenance medications facilitated by the Llanera Rural Health Unit and through the efforts of the Barangay ALDUB cordinators, who served as treatment partners of patients. Compliance to medications was attained as measured by the control of blood pressure and blood sugar level of patients as well as through a continuous community commitment for a healthier lifestyle. The whole process of creating a functional and dynamic organization catering to the needs of hypertensive and diabetic patients of Llanera, Nueva Ecija, has been a commitment of different stakeholders, most especially those of the dedicated barangay ALDUB coordinators and the rest of the whole Llanera community. | ||
