Summary: Delfin, Albano Isabela is a fourth class municipality in Cagayan Valley, Philippines. It has an integrated hospital, which is a union of the rural health unit(RHU) and municipal hospital through a Sanggunian Bayan ordinance as a way for the local government unit(LGU) to reduce cost and to maintain the function of the hospital. High rate of personnel turnover rate in the hospital and average performance in the health programs by the RHU, prompted this study which investigated the current organizational condition of the Delfin Albano integrated hospital as well as the factors affecting it, in order to recommend methods to improve its functionality. Data from document review, interviews, observer impression and survey were analysed using Weisbord's Six Box Model of organization to determine areas for improvement. Findings showed an organizational gap between the RHU and hospital with some of the staff overworked due to lack of key personnel, and leaders overburdened due to multiple role and functions. Aggravating this are the lack of governing policies on integrated hospital and the LGU's tight control on the integrated hospital of Delfin Albano, it is recommended that policies concerning structure and financing of integrated hospital be created; and managerial positions be restructured.
