Summary, etc. Clinical laboratory results are vital information that enables the physicians and other healthcare professionals to: 1. Detect disease of predisposition to disease; 2. Confirm or reject a diagnosis; 3. Establish prognosis; 4. Guide patient management and 5. Monitor efficacy of therapy. Despite the utilization of automated analyzers and rapid tests, a number of laboratory errors still occur. This definitely affects the quality of management of the clinicians. Eventually, the efficacy and efficiency of healthcare delivery is sacrificed. The scholar made an organizational assessment of the Veterans Memorial Medical Center in October 2015 utilizing the qualitative perception survey focusing on the quality of service of the Department of Pathology. Another tool used was the gathering of data from the logbooks and incident reports plus inputs from the stakeholders. The two significant findings that came out were: 1. Prolonged turn-around time of the releasing of laboratory results and 2. Erroneous laboratory results caused by incorrect/incomplete patients demographics, hemolyzed blood sample, mislabeled/unlabeled blood sample and insufficient volume of sample collected. These non-conformities fall within the Pre-analytic Phase of the laboratory examination. It affirms the findings of other laboratory experts that majority of laboratory errors arise in the pre-analytic phase because it has the most number of human interventions and processes. It is in this context that the Re-entry Project - Enhancement of the Clinical Laboratory Preanalytic Phase, was conceptualized. It seeks to enhance the preanalytic laboratory examination process by minimizing if not totally eliminating the preanalytic clinical laboratory errors as well as shortening the turn-around time of the release of the laboratory results. The end purpose is the improvement of the well-being of the sick and vulnerable. The project consists of a creation of an integrated and functioning preanalytic process/unit in the main medical laboratory of the Department of Pathology, Veterans Memorial Medical Center, North Avenue, Quezon City. It is expected to produce the following deliverables: 1. Baseline and End of Project Data; 2. Work Instructions Guidelines for every step of the preanalytic examination for the Chemistry and Hematology Units ; 3. Documented schedule of Lectures/Trainings of personnel involved in the preanalytic process; 4. A functioning structure of the Preanalytic Unit for Chemistry and Hematology Units; 6. Interfacing of the Laboratory Information System (LIS) of the Chemistry and Hematology Units with the Hospital Information System (HIS). After three and a half months of project implementation, all the five deliverables were put into effect successfully. The comparative pre-implementation and post-implementation data of the project after five weeks of execution showed a continuing decline in the preanalytic errors as presented through the quality indicators. Similarly, the turnaround time (TAT) of the preanalytic phases 1 and 2 as well as the TAT of the total testing process indicate a significant improvement. Because of the promising early results of the project, the Proponent recommended a six point agenda for the ReP's sustainability plan.
