Summary, etc. | The global situation of Millennium Development Goals 4 which is to reduce child mortality, and 5 which is to improve maternal health, is still low specially to those countries like Africa and India. In the Philippines, these MDGs is remarkably low specially in the Geographically Isolated and Disadvantaged Areas (GIDA) wherein there are no available health worker. The Province of Abra is one of the provinces in the Philippines having remarkably low MDG 4 and 5 status due to inadequacy of human resources for health. It is for this reason that the author coordinated with the Provincial Health Officers in Abra as well as to the Department of Health-Center for Health and Development for the construction of the new Rural Health Unit of Dolores. The intended purpose of the construction of the new Rural Health Unit of Dolores is for it to become a Basic Emergency Obstetric and Newborn Care (BEmONC) facility. It was constructed due to the efforts of the author and the Local Chief Executive of Dolores thru the help of the Department of Health in its Health Facilities Enhancement Program (HFEP) and the Japan International Cooperation Agency (JICA). Sine the inauguration of the new Rural Health Unit of Dolores on January 14, 2013, it started to operate as a Basic Emergency Obstetric and Newborn Care (BEmONC) facility and the RHU staff continues to attend maternal deliveries of the constituents not only from Dolores but also to nearby municipality who belong to the Inter-local Health Zone which is San Juan. The facility was certified to be a BEmONC facility on May 30, 2013 and was accredited by Philhealth for the Maternity Care Package on August 02, 2013. DOLOSAN (Dolores, Lagangilang, and San Juan) Inter-local Health Zone was created thru Executive No. 13 which has signed on the 5th day of December 2012, for the purpose of sustaining the functionality of the BEmONC facility in the three municipalities. Technical Working Group was created for the purpose of maintaining the functionality of the Inter-local Health Zone and was headed by the Local Chief Executive. It is composed of the Municipal Health Officers, and the medical officers from the community hospitals. Quarterly meetings are being conducted for the sustainability of the BEmONC facility as well as for the improvement of the health systems in those municipalities mentioned above. Any problems that arise in each of the three municipalities who belong to the ILHZ are tackled in the meetings. Sine the establishment of the BEmONC facility in the municipality of Dolores, Abra, it was found that there is a need to further strengthen the utilization of the facility. The author felt the need to create and establish support mechanisms that will sustain the functionality and will strengthen the utilization of this facility for the benefit of the constituents of Dolores. That is why he looked into parameters that should be given emphases for strengthening of the utilization of the facility. These parameters that was observed by the author as a means of evaluating the utilization of the facility are the presence of logistics/medications, adequate and competent Human Resources for Health, policy in the form of a resolution that pregnant women should only deliver in a health facility attended by a Health professional, organized patients' records and charts, financial risk protection in the form of increased coverage of enrolment to Philhealth, and a functional referral system. Fortunately, the author found that as the BEmONC facility in Dolores continues to operate, parameters that are mentioned above is said to be attained by the facility, thus, increasing its utilization. As a result, pre- and post-natal care as well as facility-based delivery increased and the occurrence of maternal and fetal death in the community was prevented due to quality heath service delivery. Furthermore, false cultural beliefs of the majority of constituents with regards to having maternal deliveries at a health facility and its effects to the mother and her baby was changes by the author which became the major contributing factor for the increase in the utilization of the facility. RHU staff also develops their confidence and had increased their competency in handling maternal deliveries because of the skills straining conducted by the author as well as the BEmONC trainings sponsored by Japan International Cooperation Agency (JICA). |
