Summary, etc. For a fifth class municipality assigned as a geographically isolated and disadvantaged area, the Botika ng Barangay provides an important alternative, and even main source of medical drugs. However, with the onset of several government initiatives and programs also aimed at improving access to affordable and safe medicines, the program is placed at a position when it needs to evaluate its role in the framework of public health delivery. The purpose of this study is to review the implementation of the Botika ng Barangay (BnB) Program in Pagsanghan Samar and develop sustainability measures customized for a geographically isolated and disadvantaged area (GIDA), while optimizing the roles of the program stakeholders. Qualitative method is applied. To facilitate review of the program, key informant interviews cross validated with document and record reviews are facilitated. In identifying an formulating local approaches for program sustainability while fortifying the involvement and support of the key stakeholders, series of focused group discussions, structured and non-structured are conducted. Strength-weakness-opportunity-threat (S-W-O-T) analysis is the primary frame of the questions used in all the instruments. The study demonstrated that the BnB program remains a needed, relevant and responsive health service, and as such entails sustainability measures especially in the setting of a GIDA municipality. While the gaps in the local program implementation are mostly consistent with the findings done on a nationwide basis save for the high influence of local politics, sustainability measures formulated should be adaptive to the local setting and must emphasize the crucial involvement and roles of the stakeholders. The findings characterized the gaps, interventions and sustainability measures to be integrative and comprehensive of these factors, namely: contextual factors, activity profile and organizational capacity. With the on-going shifts of direction of the seemingly overlapping government programs, this paper is crucial in representing one frame of the bidirectional approach of evaluating health programs.
