Summary, etc. One of the problems faced by the Municipal Health Office of Jagna, particularly RHU 2 is that cerebrovascular accidents/hypertension actually ranked 4th in the leading causes of mortality in the past 5 year average and 2012, with the rate of 56.23 per 100,000 population. According to the leading causes of morbidity in the past 5 year average and 2012, hypertension ranked 3rd. There is growing evidence that significant portion of non-communicable disease (NCD) death in the country occured prematurely. This paper aims to determine the factors, which lead to the increasing prevalence of complication (cerebrovascular accident) among hypertensive people in Mayana, Jagna, Bohol as a pilot barangay. Furthermore, measures, aligned with DOH initiative on NCD program (AO 2011-0013) are done through good clinical information system, organization support, dleivery design, decision support, and self-management support to improve patients' compliance in the management of hypertension. A policy instrument, in a form of Municipal Ordinance No. 19-11-2008 and Barangay Resolution No. 16, Series of 2014 were utilized to institutionalize active involvement of the community in the management of hypertension as a sustainability measure. Document analysis/Baseline Survey, and Interview of Key Informants/Focus Group Discussion are employed in this project to get the right perspective for collaborative effort and common understanding of various stakeholders in the use of participatory approaches in the management of hypertension. This would lead to a productive interaction between hypertensive patient and the health staff to improve their compliance to prescribed management to decrease the prevalence of complications.
