Summary, etc. In the last decade, the Philippines have been actively promoting quality and accessible maternal health in an effort to reduce maternal morbidity and mortality. Substantial progress has been made in cutting maternal mortality. It has been proven time and again that antenatal care and skilled health attendance at delivery are essential for eliminating every preventable maternal death. However, despite this progress, home based deliveries are still prevalent especially in the rural setting. Thus, this Action Plan and Project focuses on increasing facility based deliveries by intensifying pregnancy tracking. Specifically, this paper aimed to improve the current status of maternal health services in the Municipality of Makato, Aklan. It also determined approaches that can be done to establish sustainable strategies, which will increase facility based delivery and improve maternal health in the Municipality of Makato, Aklan. Document review and focused group discussions were conducted with the stakeholders involved. After which, strategies were developed to address the barriers that contribute to home based deliveries. Particularly, strengthened pregnancy tracking was conducted by barangay health workers to monitor all the pregnancies that occurred in the community. This intervention, coupled with the participation of the different stakeholders in the community, showed an increase in the number of facility based deliveries in the Municipality of Makato.
