Summary: The APP is entitled Developing a Barangay Scorecard for Health Service Delivery Performance in Dapa Surigao del Norte. Its aim is to come up with an objective appraisement tool for evaluating the health performance of the barangay based on parameters that are acceptable to the community; And to appraise the community health status using the tool developed. Specifically, the study aimed to answer the following questions: 1)What are the health services that needs to be delivered at the barangay level? 2) To what extent are theses health services reaching the barangay? 3) What can be done in order to improve health service delivery in the barangay level? and 4)What appropriate framework should be utilized in order to come up with a participatory and locally acceptable barangay scorecard? This study utilized a questionnaire developed by the collective effort of the Rural Health Unit health personnel, that was then used on the survey-forum conducted in each of the 5 identified barangays involving the (1)barangay captain, (7) barangay kagawad, barangay health workers (BHW) and (1) barangay nutrition scholar (BNS). The survey-forum served as an interactive venue to the determine the parameters that will be used for the barangay health scorecard and the willingness of the barangay to be assessed using these parameters. The data from program statistics per barangay of the different national health programs implemented in Dapa Surigao del Norte from January to Dec 2010 thru review of annual and quarterly accomplishment reports of each midwives, sanitary inspector and barangay health workers was collected and used to evaluate each of the five barangay performance on identified health parameters. That is to say these data from 2010 was used to fill up the scorecard of each of the barangay participant in the study. The study was conducted from May 2011 to October 2011 on the five barangay of Dapa Surigao del Norte namely: Cabwa, San Migeul, Brgy 12, Union and Osmena and targeted the following respondents from each of the barangay 1 barangay captain, 7 barangay kagawad, 3-4 barangay health workers (BHW) and 1 barangay nutrition scholar (BNS). In conclusion the study found out that: 1. The communities has basically correct ideas as to what health services and program should be available in the community. 2. They also recognize the need to conduct regular evaluation of their health program and health status to assess the extent of health service delivery and performance. 3. The community further see the advantages of conducting program implementation review, in particular the potential of such venture to translate into community social mobilization and its usefulness as a guide in coming up with strategies in addressing health problem and improving the health status of the community. 4.
