Summary: The goal of the action plan and project (APP) was to establish a responsive child mortality information system to help improve the survival of children in Almagro, Samar. Specifically, the study wanted to describe the current status of surveillance and reporting of under-five child mortality and identify the gaps in the implementation of a child mortality information system in Almagro. Lastly, the APP aimed to develop a policy instrument to institutionalize a child surveillance and mortality information system. The study utilized review of records from the rural health unit (RHU) and local civil registrar office. Key informant interview of the local civil registrar, RHU staff, and mothers with under-five mortality was also done. Focus group discussions and consultative meetings among the barangay health workers and Punong Barangays were lastly undertaken. The study showed that in terms of inputs, there is no local policy in support of the national framework to strengthen the registration of under-five mortality. In terms processes, there is inadequate registration coverage due to poor knowledge and behaviour of the community on the importance of death certification and registration, Finally, in terms of outputs, there is also limited venue for dissemination and use of child mortality information. In conclusion, the APP was able to assess the current status and the gaps in the implementation of the child mortality reporting system in Almagro. Building on these assessment, analysis, and consultative meetings, the child surveillance and mortality information system or CHIS,IS was developed and rolled-out to the community health workers and key barangay officials. A municipal policy was crafted and a manual of procedures was adopted to implement and institutionalize CHISMIS in Almagro.
