Summary: The objective of the study was to assess the effectiveness of HEART Program to change in the health seeking behaviour of hypertensive patients in Barangay Poblacion from November 2015 -- August 2016. Respondents were all hypertensive patients from Barangay Poblacion who were given treatment and follow-up cards, monitored blood pressure in their Barangay Health Stations weekly and had regular monthly follow-up visit in the Rural Health Unit. One hundred respondents were able to meet the set limitations of the study, thus all of them were interviewed through survey questionnaires with the help of the Barangay Health Workers. It was found out that there were more females than males among 100 respondents. This showed that gender being one of the social determinants of health affects the health seeking behaviour. Men tend not to seek out care because social norms promoted the idea that it was more masculine to not be seen as weak. The most numbered respondents belong to the 45-54 years old age range while the least numbered were from 25-34 years of age. This explains the hypertension was most frequently observed as age increases. Questions 1 and 2 in the questionnaire were used to determine health seeking behaviour. Question # 1 determines the need to comply scheduled follow-up visits while questions # 2 determines the need to compliantly take anti-hypertensive medicines. The weighted mean of both questions is within number 2. From a 4-point scoring, a low score means better health seeking behaviour.
