Summary: The current thrust of the government is to achieve UHC through PHA. The PHA values equity and inclusivity of all, transparency and accountability, efficient use of resources and provision of high-quality services. With the current situation in the persistence of inequities in health service delivery in the Philippines, achieving PHA will be difficult. Hence, a strategic approach is needed to solve the numerous challenges being encountered and achieve UHC. Sto. Nino a fifth class GIDA municipality in the province of Samar, also has numerous challenges in its health system with unsatisfactory service provision accomplishment. To improve the service delivery, the researcher focused the strategy in human health resource. This study came up with three objectives; first, to describe the current status and practices of health service delivery in Sto Nino, Samar; second, the identification of factors affecting health service delivery; and third to identify strategies in improving health service provision. The first objective was done through data review of Field Health Services Information System and Manual of Operations on Maternal, Neonatal, Child Health and Nutrition Program and the National Tuberculosis Program. Maternal services have a poor performance with the majority of indicators below the 90% target, indicators in delivery, prenatal and post partum and neonatal services. Service provision in maternal health was mostly affected by supply of commodities and stakeholder collaboration. Child services has been increasing but only with small increments. Case detection rate has been performing very poorly in the past year but has markedly improved this year due to the presence of a health worker dedicated to performing direct sputum smear microscopy. the second and third objective was performed through a focus group discussion (FGD) done by judgement sampling. Four plantilla midwives and eight DOH-HRR deployed personnel were interviewed. The FGD revealed the internal and external factors identified was the exposure to constant dangers but saving lives and helping the community still prevail with health workers still in the service. The most significant external factor identified was the poor support of the government including the barangay governance. Personality test using the Myers Briggs was also employed to determine personal factors that may affect their service provision. The MBTI result of plantilla midwives is consistent with their observed traits and attitudes. With the Villahermosa catchment poorly performing and the midwife assigned is an ESFP. It can be assumed that the personality type can explain the inefficiency observed in Villahermosa catchment. This finding is corroborated with the result in the MBTI of the midwife of Cabunga-an catchment which is the top performing catchment among the four. Strategies to improve service delivery was also gathered from the respondents. The identified strategies patterned the health human resource framework action fields: Human Resource Management, Leadership, Finance, Partnership, Education and Policy. A series of activities were conducted based on the interventions identified in each action field. Improvement in the current years in some services was attributed to the improvement in service provision of health workers by doing intensified health education and advocacy, performing trained skills and addition of health workers. Despite the intervention, the performance in majority of health programs is still unsatisfactory due to the presence of contextual factors such as poor LGU support and weak community participation. Health workers are the driver of the health system with multiple factors affecting their performance in service delivery. By focusing on the health workforce, identifying and listening to their needs, improvement in service delivery can be achieved. Health system will not only improve but we can also create dedicated vanguards of health.
