Summary: The Municipality of daram is a big island composed of 56 barangays, the primary reason why it is huge challenge for a public manager to implement a health system reform. A reform that will combat the dreaded effect of ignorance, mortality. This study aims to capacitate the overlooked human resource in the grassroots, the Community Health Team. To increase their knowledge and skill on Health Information System in order to aid the local leaders in formulating a health plan to improve the maternal health indicators. Since unfortunately, most of the barangay leaders were unaware of their health status depicted on their HIS assessment thus making them difficult to make even a simple health plan. Likewise the CHTs serve as a medium to information. They have done this through regular home visitation while assessing the family's health risks with delivery of key health messages and formulation of a health use plan per household. The action plan and project was implemented in the 4 barangays of Rizal Catchment of Daram, Samar form the month of November 2013 to September 2014. The CHTs were chosen and capacitated every monthly consultative meeting. The said meeting has the following agenda namely reporting of accomplishment for the month, discussion on issues and concerns, action planning, other matters, and scheduling and agreement of the next meeting. An assessment utilizing questionnaire and CHT rating tool was given on November 2013 and after the intervention on August 2014. This APP was able to show that with the presence of capacitated CHT on Health Information System, maternal health indicators such as ANC, FBD, SBA, and SPR improved. With the advent of poor Health Information System which was disclosed through a HIS tool on information use and dissemination, interventions such as increasing the knowledge of community health workers and motivating plus monitoring them to perform better on their recording and reporting, eventually enabled the barangay key sustainable reform in health systems in a Geographically Isolated and Disadvantaged Area like Daram. Considering that noted increase in maternal health indicators after providing HIS capacity building to CHTs, it is recommended that ALL DARAM CHTs be provide with this training per catchment. Moreover continuous and further training on HIS should be provided to CHTs in order to capacitate them into being able to assist BHBs in formulating health plans.
