Summary: Being in a fifth class island municipality where traditional healers become the primary care provider, it is difficult to give patients quality preventive care. Thus, establishing an incentive-based referral system assimilating traditional healers is a big step in changing health outcomes. This study aims to determine the effectiveness of an incentive-based referral system assimilating traditional health workers in improving referral time of patients from the community to the rural health unit. In this study, a meeting was scheduled with the traditional healers aiming to explain the referral system. After which, interested traditional healers were taught on how to fill up referral forms and on how to elicit signs and symptoms of immediately referable illnesses. Referral forms were collected and outcomes were categorized and compared to previous case studies in the municipality. Eight out of the twenty- two traditional healers were able to refer patients. Incentives will be paid in retro after the ordinance is approved. Currently the ordinance is for public hearing. Document review of patients with history of consult with traditional healers was done. The average referral time of patients before the establishment of the referral system was twelve days. After the implementation of the project, average referral time was one to two days. A comparison of cases was done per barangay as to the referral time of patients before and after the establishment of the referral system. Improvement was seen in terms of the speed of referral. In conclusion, establishing a referral system in the municipality improved referral time of patients from the community to the Rural Health Unit and assimilation of traditional healers in the referral system improved the referral system in Patnanungan, Quezon.
