There was one maternal mortality in Barangay Lagan due to severe post-partum hemorrhage. This maternal mortality was a home delivery. The goal of this action plan and project is to prevent maternal mortality by decreasing the occurrence of home deliveries in the municipality. One objective of the study is aimed to describe the current status of home deliveries in the municipality. Another objective was to identify the reasons why mothers still choose deliver at home in the municipality. Lastly, the study aims to determine the effects of intensified pregnancy tracking to the occurrence of home deliveries in the municipality. The status of home deliveries in the municipality was described by conducting documents review. Personal interviews were conducted on women who gave birth at home in 2014 and also those who gave birth at home in January to July 2015 to identify the reasons why mothers still choose to deliver at home. Intensified pregnancy tracking was done from January 2015 to the present and descriptive comparison between the percentage of home deliveries from January to July 2014 and January to July 2015 was done to determine the effects of the said intervention. Review of documents revealed persistence of the occurrence of home deliveries in the municipality. Although there is a decreasing trend of home deliveries and increasing trend of hospital deliveries from 2010 to 2014. Different reasons for delivering at home in 2014 and 2015 include: a) the mother is already in the late stage of labor and can no longer to the birthing facility, b) personal choice, c) lack of companion to bring the pregnant women to the birthing facility. Also, mothers who previously delivered at home have greater chance of having a home delivery again in the subsequent pregnancies. Majority of women who delivered at home plan to deliver in a birthing facility in their next pregnancy. But there were also women who will still choose to deliver at home. Intensified pregnancy tracking had varied effects on different barangays of Sabangan. Percentage of women who delivered at home from January to July 2015 was higher compared to the percentage of women who delivered at home from January to July 2014. Therefore we can conclude that intensified pregnancy tracking alone will not cause a general decrease in the occurrence of home deliveries in th municipality since the decision of a mother to deliver at home is affected by several socio-economic factor. The practice of intensified pregnancy tracking should be constantly reiterated and reinforced among the Sabangan Municipal Heath Office staff so that it will be norm and practiced even after the study. The community health team should also be fully utilized in pregnancy tracking. Mothers that had previous home deliveries should be closely monitored since they have a strong predisposition to have another home delivery.
