Summary: The problem of home delivery in Rosario, La Union was given priority of the author as way to decrease and prevent maternal mortality in the municipality. In Rosario, La Union, a problem in transportation was identified as one of the top reasons why mothers opt to deliver at home and not to a health facility. Increase in the number of Facility-based deliveries by means of transportation of patients to a health facility is the primary goal of this project. Monitoring and Evaluation of the project will determine the impact, effectiveness, efficiency and sustainability towards the achievement of our goal.
