The establishment of the level of knowledge of the community and the perception on this disease condition must be viewed as prerequisites to decision making, planning and interventions. The study integrated correct hypertension information through radio program and advocacy campaigns at selected barangays. Specifically, this action plan and project aimed to establish the socio demographic profile of the hypertensives, their level of knowledge and evaluate the effects of the creation and initiation of the action plan in their level of knowledge. This research project used qualitative and quantitative methods. The action plan project established the profile of the hypertensives. It shows that female population comprised the bulk and most are in their 5th to 6th decade of life with low educational attainment and income. Moreover, low level of knowledge on hypertension was predominant among them. With the creation and initiation Paspas sa Ikaayong Lawas Radio Program and Integrated Correct Community Hypertension Information for Kontrol and Aksyon (CHIKA), the level of knowledge among the target increased from a mean score of M= 29.09 to M= 43.86, from low to average level of knowledge, respectively. The standard deviation increased from a SD+ 5.15 to SD+ 9.12 which shows that mean scores are more dispersed. In conclusion, the program can be considered as success and sustainability measures are being taken to ensure the continuity of the program.
