Summary: The Re-Entry Project entitled "Assessment and Readiness Planning for Senior High School Implementation at the City Schools Division of Tacurong" was conducted from May 18 - August 18, 2015 at the City of Tacurong to ensure smooth transition from the Revised Basic Education Curriculum to the K to 12 Basic Education Curriculum particularly the Senior High School Program. This project was for the coverage of the Department of Education, City Schools Division of Tacurong, Region XII, but the methodology, if found useful, could also be utilized at a wider scale of coverage. Specifically, the project aimed to determine the level of readiness of schools for the implementation of Senior High School Program of the K to 12 in terms of teachers' readiness o handle Senior High School curriculum, classrooms and facilities and equipment essential to programs offered; to ascertain the career tracks of incoming SHS student (Junior HS in SY 2015-2016) to link them with industry needs and employment; to identify the potential problems and bottlenecks that will be encountered in the SHS implementation and how these can be addressed; to advocate for greater stakeholders' participation in the preparation and implementation of SHS component, and to determine the level of magnitude of their support; and to prepare the Readiness Plan for Senior High School implementation. Five (5) secondary schools which were identified to offer Senior High School Program were included in the project, namely: Tacurong National High School (TNHS), V. F. Grino Memorial National High School (VFGMNHS), San Emmanuel National High School (SENHS), A. S. Bernardo National High School (ASBMNHS), and Rajah Muda National High School (RMNHS). The five objectives identified to be net in the three-month implementation of the ReP had generated five (5) outputs which were as follows: Report of Current Level of Readiness of Schools for the Implementation of Senior High School; Mapping of students' SHS tracks choice; List of Bottlenecks and Problems and Recommended Solutions; Advocacy Campaign (Stakeholders' Summit) and Documentation of Commitment; and School Readiness Plans. Only one deliverable was required by the ReP Panel during the Plan Presentation last April 15, 2015 at DAP, Pasig City and this was the School Readiness Plans (SRP) which is the school's blueprint in all its actions, efforts, and initiatives in its preparation for the SHS implementation. Nonetheless, the scholar was able to produce/accomplish five outputs in the implementation of the ReP. The level of teachers' readiness to handle the SHS curriculum is Moderate while the level of adequacy for classrooms in Moderate. In terms of adequacy of facilities, materials and equipment for SHS, the level of adequacy is Low. For the ReP to be sustained. recommendations given were the following: creation of a strong, dynamic, unified and fully committed Senior High School Implementation Team for both division and school levels; utilization of the School Readiness Plan as blueprint of actions and innovations for SHS preparation and implementation; continuation of the strong and harmonious partnership with the stakeholders; and the conduct of similar study or action/ educational research on Senior High School.
